Pure Horror is an Item exclusive to Don't Starve Together. It appears to be a stronger variant of Nightmare Fuel. It can be obtained by killing a Nightmare Werepig, a Resting Horror (pure horror from resting horror drop only after Shadow Rift activation) found in the Ruins, a Fused Shadeling or by killing at least 1 of the Ink Blights.
Pure Horror is used to craft Dreadstone Helms and Dreadstone Armor, as well as every item in the Shadowcrafting Filter. It can also refuel anything fueled by Nightmare Fuel at twice the efficiency. For example, the durability of Bone Armor is restored by 50% per Pure Horror, rather than the usual 25% per Nightmare Fuel.
With the Shadow Courtier skill, Wilson can transmute 3 Pure Horror into 1 Dreadstone, as well as transmute 1 Pure Horror into 2 Nightmare Fuel.
